Case Management Social Worker – Casual

You will be joining a team of dedicated and committed youth workers whose mission is to help at-risk young people move off the streets and on to independence.

 

Summary

To oversee Case Planning and to work with Program Managers in providing comprehensive case planning to department clients while ensuring implementation of agency philosophy, policy and procedures.

 

Brief Description

  • Oversight of case management within all CHV Program & Services
  • Facilitate case planning team meetings as needed
  • Provide consultation and support to program staff regarding plans and interventions
  • Liaise with other community service providers and program services staff for the purpose of case management
  • Review and audit client files to ensure proper documentation
  • Ensure consultation of client Limitations of Service and Special Covenants
  • Provide reports on client direct service in the department as required
  • Attend Case Management Team weekly meetings to discuss common clients, incident reports, case review strategies, etc.
  • Coordinate community case conferences and internal case review meetings as necessary
  • Manage client information handling and database systems
  • Responsible for statistical data collection and overseeing of data entry
  • Collaborate on the development of forms and systems as required
  • Manage the filing and storage of all department related documentation
  • Attend inter-agency and program related meetings as required
  • Collaborate with front line staff to ensure ethical and clinically sound case plans
  • Assist and support youth workers in working with “high risk” or “special needs” clients
  • Facilitate individual client/case review meetings with each team member (monthly)
  • Provide workshops as assigned
  • Oversee and/or supervise the “learning practicum” of students in the department as assigned
  • Liaison between program and inner city youth mental health
  • Participate with Youth Workers in meeting with Youth to ensure effective action plans commensurate to case plans
  • Represent CHV in the community as necessary
  • Liaise with in-house service providers and community service providers
  • Attend community meetings and work on joint service projects as required

 

Requirements

  • Must have Bachelors of Social Work
  • Must be registered as a Social Worker (British Columbia)
  • Minimum 3 years direct service to youth population including crisis intervention and supervision experience
  • Excellent interpersonal communication skills, written and oral
  • Ability to multi-task
  • Proven case management skills
  • Case coordination with street involved youth
  • Crisis intervention skills
  • Excellent understanding of community service programming
