Social Worker



Covenant House Vancouver is the premier provider of residential and support services for our city’s homeless youth, and we are growing! You will be joining a team of dedicated and committed people whose mission is to help at-risk young people move off the streets and on to independence.

 

Job Function 

To provide direct and auxiliary social work case management services to youth at Covenant House Vancouver in keeping with agency values, philosophy, policy and procedures. 

 

Primary Duties and Responsibilities 

  • Provide case management/social work services and direct client care within all Covenant  programs  
  • Facilitate case planning team meetings as needed 
  • Provide consultation and support to program staff regarding plans and interventions 
  • Liaise with community service providers and program services staff for the purpose of case management
  • Provide thorough documentation
  • Consultation regarding client limitations and potential restrictions 
  • Provide reports on client direct service in the department as required 
  • Attend Case Management Team bi-weekly meetings to discuss common clients, incident reports, case review strategies, etc.  
  • Attend Case Review Meetings in programs on a weekly basis as requested/needed
  • Coordinate community case conferences and internal case review meetings as necessary 
  • Manage client information handling and database systems  
  • Support statistical data collection and data entry through accurate recording of data as required by case management services
  • Ensure the appropriate storage of all documentation  
  • Attend inter-agency and program related meetings as required  
  • Collaborate with front line staff to ensure ethical and clinically sound case plans 
  • Assist and support youth workers in working with youth with special or unique needs
  • Participate with Youth Workers in meeting with Youth to ensure effective action plans commensurate to case plans 
  • Represent CHV in the community as necessary 
  • Liaise with in-house service providers and community service providers

Qualifications 

  • Must have Bachelors of Social Work  
  • Needs to be able to register or be registered as a Social Worker in British Columbia with the BC College of Social Work 

Skills 

  • Minimum 3 years direct service to hard to service youth population including crisis intervention and supervision experience 
  • Excellent interpersonal communication skills, written and oral 
  • Ability to multi-task 
  • Proven case management skills 
  • Case coordination with street involved youth 
  • Crisis intervention skills 
  • Excellent understanding of community service programming

  • Schedule – Full-time, 70hrs biweekly
