Integrated Care Planning
Integrated Care Planning is informed by the Case Management team who work to provide youth-focused, coordinated, and all-encompassing services to youth.
Members of the Case Management team advocate for youth and create integrated, individual plans of care. As a result, they provide youth-focused, collaborative, wraparound services to youth.
Covenant House Vancouver uses a trauma-informed approach to caring for young people which involves integrating knowledge of trauma into all aspects of service delivery.
The Case Management team focuses and builds upon the unique gifts and strengths that the youth at CHV possess.
Activities include:
- Conducting assessments to gauge a youth’s overall health (mental, physical, and spiritual) as a starting point for identifying where they are at, and how to structure care moving forward.
- Conducting regular case plans for the purpose of short- and long-term goal setting.
- Referrals to, and coordination of, specialized services like psychiatrists and other medical professionals, including partnerships with Vancouver Coastal Health and Foundry (Providence Healthcare).
- Collaborating, both internally and with external organizations, along with case reviews and community meetings
- Reflective practice and consistent professional development to better work with, and engage with, youth.
- Running groups like art therapy, grief and loss discussions, and taking youth for recreational activities.
