Integrated Care Planning

Integrated Care Planning

Integrated Care Planning is informed by the Case Management team who work to provide youth-focused, coordinated, and all-encompassing services to youth.

Covenant House Vancouver takes a trauma-informed approach to caring for our young people which involves understanding, recognizing, and responding to the effects of all types of trauma.

The Case Management team focuses and builds upon the unique gifts and strengths that the young people at CHV possess.
