Youth Worker – Outreach (T-F)

Covenant House Vancouver is the premier provider of residential and support services for our city’s homeless youth, and we are growing! You will be joining a team of dedicated and committed people whose mission is to help at-risk young people move off the streets and on to independence.

The Outreach Youth Worker is responsible for the direct implementation of Covenant House Vancouver’s philosophy, policies and principles as they pertain to client care and safety to all.  The Outreach Youth worker’s primary role is to build therapeutic professional relationships with youth. Youth Workers will collaborate with other agency staff to determine the best plan of care to help youth reach their goals and transition to healthy, confident adults.


Job Description

Direct Service to Youth

  • Assist youth to complete intake assessment forms upon intake to the Community Support Services Program
  • Assist youth in accessing harm reduction supports and services
  • Initiate, develop and maintain relationships with youth 16-24 years of age
  • Assess the needs of the client and establish plans of care for clients
  • Assess clients in identifying attainable short and long term goals, and provide an action plan for implementation
  • Assist clients in exiting street life, maintaining independence or accessing supports to have their needs met
  • Accompany youth to scheduled appointments and support youth in crisis
  • Coordinate access to housing workers for youth moving into their own accommodations or requiring housing support
  • Provide Outreach support as required to assist youth in meeting their basic needs
  • Provide Outreach support as required to assist youth with maintaining housing and independent living
  • Provide Outreach support and referrals to clients with addictions, mental health and behavior management issues,
  • Provide information and guidance to support youth looking to better their education and employment goals
  • Provide the basic needs and services to those youth residing  outside the Covenant House continuum of care

Case Management, Documentation and Other Duties 

  • Ensure information regarding ETO (Efforts to Outcomes) is entered daily including daily efforts, drop in and outreach statistics
  • Document client notes in ETO,  write Incident Reports, and other routine documentation as required
  • Read Drop in Communication Log and Incident Reports daily
  • Administer , fare savers and taxi vouchers for youth seeking assistance with transportation needs
  • Record daily attendance of youth entering the CSS drop in space into the ETO data base system

Job Requirements

  • Diploma or degree in related social service
  • Valid BC drivers license with no restrictions
  • Minimum of 2 years related experience
  • Excellent interpersonal and communication skills
  • Non-violent crisis intervention / managing hostile interactions certificate a strong asset
  • Valid first aid certificate
  • Outreach workers are required to carry backpacks in excess of 30lbs
  • Able to walk long periods of time in varying weather conditions
  • Limited access to elevators
  • Familiar with runaway and street involved youth issues
  • Leadership and team building
  •  Basic computer skills

Additional Information

  • Department – Community Support Services
  • Schedule- Tuesday to Friday 9am – 6.30pm. Hours vary depending on youth needs
